The Community Health Worker (CHW) will support Community Wellness Outreach initiatives by enhancing care coordination, addressing social determinants of health (SDOH), closing care gaps, and improving patient care. The CHW will play a critical role in improving patient outcomes, reduce readmissions, and reduce health disparities across Calvert County.

High school diploma required. Registration/Certification/Licensure : Maryland Community Health Worker Certificate required
