• 4:30pm LOCATION Gateway Plaza/University at the Coleman Center KEY RESPONSIBILITIES Provide outreach, education, and ongoing support to pregnant individuals and families throughout pregnancy, labor, and the postpartum period. Serve as a trusted advocate and bridge between the community and healthcare, government, and social service systems. Assist clients in identifying and overcoming barriers to accessing healthcare and community resources. Connect individuals and families to essential services, including: Prenatal and postpartum healthcare Health insurance enrollment and navigation Food assistance programs Housing resources Transportation services Childcare and family support programs Community-based health and wellness services Conduct assessments to identify client needs and develop appropriate resource and referral plans. Provide culturally responsive health education related to prenatal care, maternal health, infant care, and healthy lifestyles. Support clients in navigating healthcare systems, scheduling appointments, and understanding recommended care plans. Offer informal counseling, encouragement, emotional support, and follow-up services to clients and families. Advocate for clients to ensure access to quality healthcare and supportive services. Maintain regular communication with clients to monitor progress and address emerging needs. Collaborate with healthcare providers, community organizations, and social service agencies to coordinate care and services. Facilitate referrals and ensure successful connections to community resources and support networks Maintain accurate documentation of client interactions, referrals, services provided, and outcomes. Participate in community outreach events, educational programs, and health promotion activities. Promote positive maternal and infant health outcomes through education, advocacy, and resource coordination. Support family well-being by encouraging self-sufficiency, healthy behaviors, and engagement with available community resources. REQUIREMENTS High School Diploma or equivalent required. Must obtain a Community Health Worker certification within 1 year of hire, in accordance with state and organizational standards. Ongoing continuing education may be required to maintain certification. 0
