Community Health Worker at Providence Santa Rosa Memorial Hospital in Santa Rosa, CA. This position is Full time and will work 8-hour, Day shifts. The Community Health Worker (CHW) is a trusted community member who acts as a liaison between a community and health/social service systems.

1-3 years relevant community experience Within 6 months (180 days) of hire: Complete the Community Health Worker Training and be certified by the state of Oregon Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request.
