Works as a liaison to grant support to the vulnerable members of the community, addressing material and health challenges. Maintains a caseload of members and provides referrals to various support services by conducting assessments and in home visits. Serves as a role model and will identify and initiate referrals to social service programs, serve as main point of contact for the member and support the implementation of various care activities.

Works to establish positive and support relationships and effective works with a diverse group of members. 10. Delivers coaching and mentorship on chronic health conditions and self-care. 11. Enters client data into software on a daily basis. 12.
