About the role

Responsible for working closely with individuals and communities to promote and maintain good health. Responsible for providing education, support, and outreach services to community members to help them achieve their health goals. Areas could include exercise, mental health, medication management, nutrition, health care system navigation, substance use and other health behaviors.

What you’ll do
Conduct community outreach andConduct community outreach and engage community members to understand their health needs
Provide individual and groupProvide individual and group health education and coaching to community members
Assist community members inAssist community members in accessing and navigating the healthcare system, including providing referrals to appropriate health services
Provide basic health screeningsProvide basic health screenings and assessments to identify health risks and provide appropriate interventions
Assist in the developmentAssist in the development of health promotion programs and activities to address community health needs
