About the role

As a Community Health Worker, you will work directly with individuals and families throughout the NVRH service area to assess needs, identify resources, and support clients in achieving improved health and well-being. You will collaborate closely with healthcare providers, community organizations, and members of the Community Health Team to ensure coordinated, patient-centered care.
