Work Shifts 9:00 A. M – 5:00 P. M Monday through Friday 35 hours Alternating weekends.
Duties & Responsibilities SUMMARY OF DUTIES AND
- Under the direction of Pediatrics Primary Care leadership, the Community Health Worker (CHW) both provides direct support to patients, parents, and caregivers around developmental, behavioral, and health-related social needs, and assists with supervision and training of more junior CHWs. This position is part of TeamUp, a grantfunded initiative integrating behavioral health into pediatric primary care from birth through adolescence. Candidates are expected to take part in TeamUp activities and demonstrate strong collaboration and communication skills to function effectively as part of an interdisciplinary care team. They play a critical role in improving quality of care for our System's patients through oversight of community outreach activities that connect patients and families with resources such as community and parenting services, mental health support, and prenatal care. All references to patient below may include their parents and/or caretakers, where appropriate. General tasks and responsibilities will include:
- Partner with CHW lead and clinical leadership to refine and deliver TeamUp’s CHW Program to support specific facility, department and patient needs.
- Participate in regular meetings with TeamUp and clinical leadership to review program progress
- Assist in recruitment and hiring of CHW candidates, as needed.
- Assist CHW lead with training and mentoring process for CHWs, including the development of individualized learning plans to meet the specific professional development needs of a workforce with diverse backgrounds.
- Engage and recruit eligible patients and conduct clinical or community-based assessments and interviews to identify their health goals and needs.
- Assess and address health, social and service-related needs of assigned/identified patients and create individualized, patient-centered, goal-directed care plans for each patient.
- Utilize evidence-based engagement strategies, such as motivational interviewing (MI) and problem-solving techniques, to engage and maintain trusting relationships with patients and caregivers, and to assess for and monitor emerging needs for prevention, wellness, medical, and behavioral health care.
- Escalate issues to Clinical Leadership as appropriate.
- Participate in weekly case conferences with the CHW team to review goal-directed interventions and work with CHW team to refine patient recruitment strategies to ensure enrollment targets are met.
- Assist with facilitation of communication between CHWs and clinic and hospital care teams; educate clinical teams on CHW program.
- Manage patient outreach list and track enrollment progress using the electronic health record (Epic) and other programs, as needed.
- Stay up-to-date on, and ensure CHW team is informed of, evolving patient resources including hospital resources, community services, and benefit programs.
- Monitor and track program progress and effectiveness of CHWs
- Carry out CHW program quality improvement initiatives.
- Participate in ongoing training activities related to the community health worker role and program, as well as public health emergency response skills relevant for the TeamUp program .
- Perform other related program tasks, as needed and perform duties in response to public health, emergencies, when necessary.
Assignment Qualification
1. Four (4) years of full-time experience in counseling, community work or community health activities in a government agency or community organization engaged in providing community services to the public, assisting members of the community in obtaining community services or maintaining liaison with schools, community organizations or other government agencies for the purpose of providing assistance and obtaining participation and support for implementation of community or public service programs; or 2.
Satisfactory combination of education and/or experience equivalent to 1 above. Study at an accredited college or university in Sociology, Psychology or other Behavioral Science discipline may be substituted for experience on the basis that thirty (30) semester credits are equivalent to one (1) year of experience. However, all candidates must have at least one (1) year of full-time experience described in 1 above.
Additional Assignment Education Preferences: 1. Preference will be given to applicants with at least a high school diploma or its educational equivalent approved by a State’s Department of Education or a recognized accrediting organization. Preferred Knowledge Areas, Skills, Abilities, and other Characteristics: 1.
Familiarity with email, word processing, and internet usage and comfort learning new computer programs. 2. Experience Using the Following Software: Microsoft Office (e.g., Word, Excel, and PowerPoint), Electronic Health Record, Epic, FindHelp, MyChart. 3. Must possess excellent communication, documentation, reading and writing skills. 4.
Proficiency in a second language is preferred (e.g., Albanian, Arabic, Bengali, Cantonese, French, Haitian Creole, Hindi, Korean, Mandarin, Russian, Spanish, Urdu). 5. Experience supervising staff in a health care setting, non-profit, or community-based organization; comfort supervising staff both in- person and remotely. 6. Ability and willingness to conduct home visits and outreach activities with assigned CHWs, as needed. 7.
