✓High School Diploma.
Associates Degree in Health Services relate field preferred.
Qualifications
✓At least two years of experience in Human Services field and/or working with parents of infants and toddlers.
✓We take great care of careers.
Use of hospital EPIC systems, Parents as Teachers curriculum, providing breastfeeding education and support. Familiar with Community Based Services in designated region preferred.
Bilingual- Fluent in English and Spanish preferred.

About the role

The Connections Coordinator supports the Stronger Families, Stronger Future (SFSF) grant-funded program administered by the Connecticut Office of Early Childhood. The Connections Coordinator screens families in the prenatal and/or postnatal period using a standardized tool to help decide which of the three program components best meets the needs of each family.
