About the role

This role focuses on building life skills, coordinating essential services, and fostering community integration—both at home and in the community.

What you’ll do
Coordinate and facilitate servicesCoordinate and facilitate services that support clients’ ability to live independently in their family home
Assist clients and familiesAssist clients and families in navigating and accessing community resources and “generic services.
Provide support and trainingProvide support and training in daily living skills, both in-home and in the community
Communicate regularly with familiesCommunicate regularly with families to ensure alignment and progress
Help schedule and coordinateHelp schedule and coordinate medical and other essential appointments
