M Monday - Friday Duties & Responsibilities Purpose of Position: Under direction of the Chief Nursing Officer, with the broadest scope for the exercise of independent initiative and judgment, organizes, directs, coordinates and evaluates the nursing service and nursing education programs of a health care setting or diagnostic and treatment center; performs related tasks.

1. Develops, establishes and implements the philosophy, objectives, policies and standards of the Nursing Department. 2. Organizes the Nursing Department to delineate authority, functional responsibilities and lines of communication.

Coordinates functions of nursing services with other disciplines, collaborates with administrative staff, other department staff and representatives of allied groups in planning services. 3. Determines staffing requirements in relation to patient care standards and develops procedures to maintain an adequate, competent nursing staff. 4. Maintains a system of continuing evaluation of nursing service provided for patients. 5.

Maintains up-to-date policy procedure manuals for all nursing units and evaluates their effectiveness. 6. Establishes an effective system of clinical and administrative nursing records and reports. 7. Directs the preparation of the personnel, equipment and supply budgets for the Nursing Department. 8.

Directs the development and implementation of orientation, training and continuing education programs for nursing personnel. 9. Initiates and participates in studies and research projects. Utilizes findings to improve patient care and administrative services. 10.

Plans for and coordinates use of clinical facilities by colleges and other agencies for nursing education purposes. 11. Participates in the development of policies and health programs for the health care setting and System policies and programs involving nursing. 12. Develops and maintains sound relationships with the community, labor and professional groups in developing nursing programs and policies. 13.

May serve as a consultant for the System for the overall nursing practice, service and education programs; conducts related studies and directs projects, provides leadership in Central Office in the development, coordination and evaluation of System’s central and health care setting nursing services, practice and education programs. 14. Attends community conferences and committee meetings with administrative, Medical Board and department heads. 15.

Conducts and chairs staff meetings and working committees with all levels of nursing personnel.

1. Licensed and currently registered to practice as a Professional Nurse in New York State; and, 2. A Master's Degree in Nursing Administration or an equivalent field, and a minimum of four years of high-level responsible experience in nursing administration, three years of which shall have been in a hospital; or, 3.

A satisfactory equivalent. However, all candidates must be licensed and currently registered to practice as a Professional Nurse in New York State. Department Preferences Clinical Services Duties and Areas of Responsibilities Develop and review metrics that lend to the growth of the CHHA and management of staff productivity and work performance.

Hires, trains and develops staff based on the growth of the program and operational needs of the organization. Oversees the daily functions and activities of the CHHA Nursing Service Delivery team. Liaises with acute and post-acute care discharge planning teams to ensure transition goals for patients are met.

Thinks strategically in planning for referral sources , patients and the home care department to improve on the quality of care being rendered. Assures adequate supply of qualified staff based on operational needs and productivity targets. Assists with recruiting , hiring and orienting all clinical staff.

Provides one to one mentoring and education to direct reports as needed. Provides adequate opportunity for education and ensures that all staff meets mandatory educational and in-service requirements on a timely basis. Contributes to the overall operations and strategic planning for the growth of the Certified Home Health Agency.

Oversees the clinical staff assigned to the various CHHA Teams to establish and maintain positive, productive relationships with facility stakeholders and to support effective patient support. Thinks strategically for our patients and caregivers, leveraging technology to streamline processes and improve quality of care. Works closely with Directors of Clinical and Community-Based Services and TCNs to ensure compliance with home care regulatory guidelines, review "safe" discharge planning, payer limitations and restrictions.

Assumes on-call responsibilities as required and serves as back up to Assistant Directors of Nursing (AON) for Service Delivery - Clinical Services. Manages schedule of ADNs for Service Delivery- Clinical Services and ensures team coverage in the absence of the ADN for Service Delivery - Clinical Services. Ensures that staff is counseled appropriately and timely with regards to work performance, attendance, punctuality, patient care quality and productivity issues.

s annually, and when necessary. Ensures availability of structured onboarding plans for all assigned personnel. Facilitates positive interpersonal relations among staff with a focus on promoting and modeling ICARE values in all engagements .

Ensures that the agency is following all Federal, State and accrediting body regulations. Participates in short/long-range strategic planning and implements specific and targeted measures for growth. Establishes work-flows as needed to stream-line processes that will enhance operations and work- performance.

Contributes to the development of marketing strategies and participates in programs aimed at improving the visibility of the home care department. Strengthen relationships with new and existing patient care providers by daily or weekly follow- up. Conduct ongoing assessments to ensure ADN for Service Delivery - Clinical Services presence, engagement and interaction within their assigned teams and direct reports and make changes when needed.

Conduct regular meetings to facilitate the dissemination of information and maintain the team's ongoing connection within the Community Care network. Assists with community relations and community education. Travel between multiple post-acute and acute care sites required daily.

Provides leadership by satisfactorily managing relationships with and among customers (patients/clients, medical staff , volunteers, vendors, colleagues) Demonstrates a capacity for accountability. Supports all appropriate hospital programs, including mandatories. Develops goals, and objective and establishes plan to accomplish responsibilities. Determines sources of conflict and responds timely to achieving a resolution.

Skillfully and even handedly administers the human resources process (hiring, training, evaluating, developing, coaching and discipline) as related to the agency's policies and procedures.

Education and Learning Administration Oversee the evaluation and learning needs of CHHA staff. Provide clinical expertise and implement competency-based orientation tools, and training to improve quality. Home Care Scheduling Oversee daily operations of the Central Scheduling Team to ensure timely, accurate scheduling of patients to field staff, and exceptional customer service in alignment with clinical priorities and patient needs Establish and maintain scheduling protocols to optimize efficiency, equity in staff assignments, and continuity of patient care. Minimum of 3 years high-level responsible experience in nursing administration Minimum three 2-3 years of experience in home care Minimum of 1 to 2 years’ experience in an acute care or in- patient post-acute care setting. Minimum 1 to 2 years’ experience in discharge planning preferred. Minimum 1 to 2 years’ experience in business development preferred.

Previous experience using various computer/mobile software applications: i.e., Outlook, Word, Excel, PowerPoint, Visio, OneNote. MINIMUM KNOWLEDGE, SKILLS. AND ABILITIES REQUIREMENTS Incorporates knowledge of cultural differences through effective instructional interactions.
