Licensure
✓Valid license and current registration to practice as a Pharmacist issued by the New York State Education Department (NYSED)
✓Valid license and current registration to practice as a Pharmacist issued by the NYSED
Education
✓and completion of a Doctor of Pharmacy (Pharm.D.) program or Master’s Degree from an accredited college or university in Business Administration, Public Health or a related discipline
Qualifications
✓and four (4) years of satisfactory, full-time experience as a Licensed Pharmacist, two (2) years of which must have been in a responsible administrative, managerial or supervisory capacity in a Pharmacy in a hospital, health or medical care setting
✓and seven (7) years of satisfactory, full-time experience as a Licensed practicing Pharmacist, three (3) years of which must have been in a responsible administrative, managerial or supervisory capacity in a Pharmacy in a hospital, health or medical care setting.
