Under the direction of the Supervisor of Care Continuum and collaboration with the bedside Case Management team, processes referrals for patients services to meet their discharge planning needs, assures a cost and quality effective discharge, and always promotes patients' rights and maximum benefits from health care services. This position is part time, working Monday - Friday, 6 hr days 8am-2pm or 10am-4pm. Position works 60 hours in a 2 week pay period and is benefits eligible.
One year in the medical field (i.e. hospital, home health or nursing home). Prefer on year discharge planning or processing referrals. Insurance coverage exposure is a plus.
SPECIAL
Excellent communication, interpersonal, and organizational skills. OTHER: Computer skills including Microsoft Office and electronic health records software.
Preferred: Bachelors - Related Field
