✓BLS required.
Education
✓High School Diploma is required.
Junior College/Technical School preferred.
Qualifications
✓Working knowledge of medical terminology. Excellent communication skills-verbal and written ability to work with patients, families, physicians, and other hospital departments.
Education and training in hospital healthcare transitional care, and discharge planning beyond the hospital setting, preferred.
About the role
Mary's Medical Center is seeking a full time Discharge Planning Specialist. The Discharge Planning Specialist (DPS) will assist the Care Facilitator and or Social Worker with the discharge planning responsibilities, to promote timely and safe discharge services.
