✓Current, unrestricted RN license in the State of Texas
Certifications
Case Management experience including Certification in Case Management preferred
Qualifications
✓3+ years clinical experience in a hospital, acute care, home health/hospice, direct care or case management
✓Proven ability to communicate complex or technical information in a manner that others can understand, as well as ability to understand and interpret complex information from others
✓Proven ability to be flexible with daily schedule
✓Proven problem-solving skills; the ability to systematically analyze problems, draw relevant conclusions and devise appropriate courses of action
✓Computer / typing proficiency to enter / retrieve data in electronic clinical records; experience with email, internet research, use of online calendars and other software applications
✓Reliable transportation to travel within service delivery area which will be up to a 50 mile radius from your home
✓All employees working remotely will be required to adhere to UnitedHealth Group's Telecommuter Policy
Experience working with the needs of vulnerable populations who have chronic or complex conditions preferred
Medicaid, Medicare, Managed Care experience preferred
Oncology experience preferred
Home care / field based case management experience preferred

About the role

As a Field - Based Case Manager, you'll wear many hats and you will be visiting members in the hospital before discharge possibly with new medications or diagnoses. You'll assist members with safe, effective transitions from care environments to where they live.
