Qualifications
✓2+ years of clinical experience as an RN or LBSW
✓1+ years of experience with MS Office, including Word, Excel, and Outlook
✓Excellent organization, communication and time management skills
✓Reside in or within a commutable driving distance to Wilson, Smith, and Davidson Counties
✓Access to reliable transportation and valid US driver's license with the ability to travel up to 50% within an assigned territory to meet with members and providers
✓All Telecommuters will be required to adhere to UnitedHealth Group's Telecommuter Policy.
1+ years of community case management experience coordinating care for individuals with complex needs preferred
Long term care experience preferred
Experience working in team-based care preferred
Working knowledge of Medicare/Medicaid regulations preferred

About the role

The primary purpose of this position is the application of clinical knowledge to coordinate member transitions from nursing facility to community or between community-based settings for assigned members who are primarily medically complex and require intensive medical and psychosocial support. The Transition Care Coordinator works directly with members to ensure appropriate access to services and providers.
