✓Requirements
Licensure
✓Licensed or eligible for licensure as a Registered Pharmacist (R.Ph.) in the State of Ohio required.
Education
✓Doctor of Pharmacy (PharmD) required.
Completion of a PGY1 Pharmacy Residency preferred.
Qualifications
✓Strong clinical knowledge of cardiovascular and heart failure pharmacotherapy.
✓Expertise in therapeutic drug monitoring and medication optimization.
✓Knowledge of guideline-directed medical therapy for heart failure and other cardiovascular disease states.
✓Excellent communication and interdisciplinary collaboration skills.
✓Ability to analyze complex clinical data and provide evidence-based recommendations.
✓Demonstrated commitment to teaching, quality improvement, and advancing patient care.
Completion of a PGY2 Cardiology, Critical Care, or equivalent experience strongly preferred.
Advanced training or experience in cardiovascular pharmacotherapy preferred.
Experience caring for patients with cardiovascular disease, acute decompensated heart failure, or critically ill cardiac patients preferred.
Experience with multidisciplinary cardiology and/or critical care practice models preferred.
Experience with quality improvement initiatives and guideline implementation preferred.
less than 1 year Experience as a pharmacist with training in drug therapy of diseases including the knowledge and application of clinical pharmacology, pharmacokinetics, drug-drug interactions, and pharmaceutics to patients, training in cost analysis/cost evaluation techniques (Preferred)
Computer systems skills in spreadsheet, database and word processing (Preferred proficiency)
