✓Requirements
Licensure
✓Current unrestricted Vermont Licensure in applicable clinical discipline.
Certifications
✓Maintain satisfactory standing with the Adult Abuse Registry, Child Abuse & Neglect Registry, Vermont Criminal Information Center and OIG
Education
✓Bachelor's Degree in Nursing, Occupational Therapy, Physical Therapy or other relevant health care discipline required.
Master's Degree in Nursing, Rehabilitation Services, Public Health, Healthcare Administration, Social Work, or related field preferred.
Qualifications
✓Valid driver's license and reliable transportation.
✓Five or more years of progressively responsible clinical experience in pediatrics, family health, rehabilitation, home health.
✓Three or more years of leadership, supervisory, or program management experience.
✓Experience in care management, case management, population health, transitional care, community health, chronic disease management, or value-based care models.
✓Knowledge of healthcare reimbursement models, care coordination practices, and community-based service delivery.
✓Demonstrated experience leading interdisciplinary teams and managing complex patient populations.
✓Strong understanding of quality improvement methodologies, utilization management, and performance metrics.
✓Experience developing community partnerships and working collaboratively with hospitals, primary care practices, behavioral health providers, Childcare providers and community agencies.
✓Excellent communication, relationship-building, and conflict-resolution skills.
✓Ability to analyze data and use information to drive operational and clinical improvements.
✓Strong organizational skills with the ability to manage multiple priorities and changing demands.
