Home Health and Hospice Aide (CNA) Announcement #342326 Jefferson Healthcare is currently looking for a full-time Home Health and Hospice Aide (CNA) to join our team. We have a fully accredited Home Health & Hospice program and this role will involve providing direct personal care to patients in their homes. You will be following a specific plan of care prepared by the RN Case Manager to ensure that patients receive the best possible care.
