Join Elara Caring as a Home Health RN Case Manager serving patients throughout Boston Proper, including Chinatown . You’ll provide skilled care in patients’ homes and help them recover, manage their health, and stay independent.

That includes providing skilled nursing visits, developing and updating care plans, managing an assigned caseload, and coordinating with physicians and other clinicians. You’ll also teach patients and caregivers how to manage medications, treatments, and health conditions at home. What You’ll Bring Current, unrestricted Massachusetts RN license At least one year of clinical nursing experience Strong assessment, communication, and time management skills Ability to work independently and travel throughout Boston Proper, including Chinatown Valid driver’s license, current auto insurance, and reliable transportation Ability to sit, stand, bend, lift, and move intermittently and lift 50–100 pounds Ready to bring your nursing skills into the homes and neighborhoods of Boston?
