Duties & Responsibilities May be asked to do client admission on occasion. Regularly evaluate the patient’s nursing needs. Be available to take call after working hours.
Make necessary revisions to Plan of Care. Provide services requiring a specialized nursing skill. Provide appropriate palliative and comfort care.
Prepare clinical and progress notes. Assist with coordination of services of Home Health Aides, Homemakers, PT, OT, Social Worker, Spiritual Caregiver and other necessary services. Inform the physician and other personnel of changes in the patient’s condition and needs.
Counsel the patient and family in meeting nursing and related needs. Participate in in-service programs as directed. Make a supervisory visit to the patient’s residence at least every two weeks when the Home Health Aide is present to observe, assist, and assess relationships and determine whether goals are being met.
Available at all times during operating hours and participate in all activities relevant to the professional services provided, including the development of qualifications and assignments of personnel. Attend weekly team meetings when appropriate. Qualifications Qualifications Graduate of an accredited School of Nursing.
Current RN license in the state of Minnesota. Ability to maintain good working relationships with staff. One year of nursing experience preferred.
Must have a valid Minnesota driver’s license. Knowledge, Skills & Abilities Ability to read and communicate effectively. Strong written and verbal skills.
Basic computer knowledge. Decision-making and problem-solving. Team building.
Performance Standards Must adhere to HIPAA confidentiality standards. Must adhere to the Keys to Heartfelt Care. Must be able to perform the physical requirements of the position.
