What We OfferJoin Novant Health as a Hospice Liaison Nurse and serve as a vital connection between patients, families, healthcare providers, and hospice services. In this highly collaborative role, you'll help ensure timely access to hospice care by coordinating referrals, conducting clinical assessments, facilitating admissions, and supporting seamless transitions from acute care settings to hospice services. The primary focus of this role is referral processing and admission scheduling, ensuring eligible patients receive the right care at the right time.

You'll partner closely with physicians, case managers, discharge planners, social workers, and healthcare teams to provide education, advocacy, and support throughout the hospice transition process. Process incoming hospice referrals and coordinate admission schedulingConduct comprehensive clinical assessments to determine hospice eligibility and patient needsFacilitate hospice admissions and ensure smooth transitions from hospitals and other healthcare settingsCollaborate with physicians, case managers, social workers, discharge planners, and interdisciplinary healthcare teamsEducate patients, families, and referral sources on hospice services, benefits, and eligibility requirementsSupport discharge planning efforts and help remove barriers to timely hospice placementBuild and maintain strong relationships with hospital staff, providers, and community partnersEnsure compliance with hospice regulations, clinical standards, and organizational policiesAdvocate for patients and families by promoting comfort, dignity, and quality of lifeCoordinate care to ensure exceptional patient and family experiences throughout the transition processWork EnvironmentPrimarily hospital-based position serving as an on-site liaison between Novant Health hospitals and hospice servicesProvides coverage across five Novant Health hospitals based on business and patient needsOccasional field-based admissions and patient visits may be requiredDynamic environment requiring strong communication, organization, and relationship-building skills What We're Looking ForEducation: 4 Year / Bachelors Degree, preferred.

Graduate of an accredited school of nursing required; BSN preferred.

Minimum three years' experience in home care or hospice, required. Previous experience in managed care, infusion care, and case management and previous home health/hospice case management experience, preferred. Licensure/Certification: Current RN licensure in appropriate state, required.

Additional Skills (required): Excellent nursing assessment skills/judgment and understanding of Hospice philosophy. Knowledge of Hospice services necessary to obtain and interpret information in terms of the patient’s needs in order to appropriately receive referrals. Knowledge of Medicare/Medicaid rules/regulations necessary to understand criteria for determining patient eligibility for admission.

Knowledge of nursing and pharmacy procedures and standards of practice, both general and specific, to the specialties of infusion and enteral therapies. Strong writing skills, organizational and analytical skills. Must be flexible, adaptable to change and able to apply problem-solving skills and critical thinking skills.

Ability to interact effectively with diverse individuals at all levels of authority, in teams and on committees. Must have computer experience and have reliable transportation to and from all work related assignments and activities. Must be flexible to travel as needed between different locations, as assigned, in all weather conditions and on roads in varying degrees of repair.

Ability to work in environment containing distractions and interruptions such as noise and telephone calls. Ability to handle varying and unpredictable situation such as emergencies or crisis situations. Must be available for after hours call on rotational basis.
