Hospice LPN Continuous Care (PRN)

**This position is based in Hialeah, FL and requires daily travel within a assigned territory in Miami Dade County.

Collaborate with physician and RN frequently to ensure patient needs are being met; Notify them as soon as possible when patient condition changes. Teach patients and caregivers self-care techniques. Check vital signs.

Review all medications and perform a medication reconciliation at each visit. Perform specialized procedures ordered by physician (i.e. foley catheter change). Prepare equipment and materials for treatments using the aseptic technique.

Complete documentation timely and accurately. QualificationsBilingual in English/Spanish REQUIREDOne year of clinical LPN experience REQUIREDCurrent LPN license, specific to the state(s) you are assigned to workCurrent CPR certificationValid driver's license, reliable transportation, and liability insurancePreferred:Hospice or home health experienceStrong communication and interpersonal skillsAbility to work independently and manage multiple prioritiesComfortable with daily travel between facilities and patients homesProficient in electronic medical record (EMR) systemsReady to take the next step?
