Licensure
✓Board Certified or Board Eligible in Hospice & Palliative Care or having obtained the Hospice Medical Director Board Certification (HMDCB)
✓Maryland Medical License, CDS & DEA licenses (or obtainable)
Certifications
✓Prior to a patient's third and subsequent re-certifications, ensure that clinical findings support continued eligibility for hospice care, including performing any needed face-to-face patient care evaluations.
Education
✓BridgingLife is accredited by the Community Health Accreditation Program (CHAP).
Qualifications
✓MD or DO from an Accredited Medical School
✓CHAP accreditation publicly certifies that an organization has voluntarily met the highest standards of excellence for home and/or community-based health care.
✓BridgingLife is a member of the Hospice and Palliative Care Network of Maryland and the National Hospice and Palliative Care Organization.
Hospice and/or Palliative Care physician experience is preferred
