Find Your Passion and Purpose as a Hospice Visit PhysicianLocation: Farmington, CTCoverage Area: Northern ConnecticutSalary: Per visitSchedule: 3-4 hrs/wk, PRN What You Need to Know AccentCare is seeking a hospice field visit physician that can provide compassionate, patient-centered medical care to individuals with serious, advanced, or terminal illnesses. This role is responsible for conducting hospice and/or palliative care visits, evaluating patients for hospice eligibility and recertification, managing pain and symptoms, and supporting patients and families through goals-of-care and advance care planning discussions.

If you meet these qualifications, we would love to meet you: Must be a licensed Medical Doctor (M. D.) or Doctor of Osteopathy (D. O.) in the state(s) of practice, with training or experience in caring for chronically or terminally ill patientsMaintains active, unrestricted medical and DEA licenses in the state of ConnecticutWorking knowledge of hospice and palliative care, including pain and symptom management.

Familiarity with hospice regulations and Medicare requirements. Strong clinical judgment and decision-making skills. Excellent communication skills for interaction with patients, families, and providers.

Ability to manage complex clinical and ethical situations in end-of-life care. (Preferred) Hospice or hospice visit experience

Evaluate patients for pain, symptom management, and hospice eligibility (admissions and recertifications). Counsel patients and families on care options and advance care planning. Order tests, initiate treatment, and conduct face-to-face visits as needed.

Serve as Attending Physician when appropriate. Coordinate care with interdisciplinary teams and communicate with attending physicians to ensure continuity. Manage patient medications.

Participate in on-call rotation and provide 24/7 availability when assigned. Complete timely and compliant clinical documentation. Why AccentCare?
