Certifications

CNA, HUC, or other relevant healthcare training or certification preferred
Education
✓Associate degree in healthcare, human services, business, or a related field, or an equivalent combination of education and relevant experience
Bachelor’s degree preferred
Qualifications
✓Previous experience in healthcare, patient-facing care, care coordination, or administrative support
2+ years of healthcare experience preferred
Experience in hospital care management, care coordination, discharge planning support, health unit coordination, medical assisting, patient care, or a related setting preferred

About the role
In this role, you’ll partner with RN and Social Work Care Managers to coordinate referrals, post-acute and community resources, insurance authorizations, patient and family communication, documentation, and data collection. You’ll collaborate with the interdisciplinary care team to identify and escalate barriers that could delay a patient’s transition from the hospital.
