Job Summary and Qualifications R esponsible for planning and coordination of appropriate patient placement based upon diagnosis, special needs and acuity Establishes and maintains collaborative practice and working relationships with all healthcare team members and serves as a liaison for patients, families, physicians, and nursing thr oughout the placement process Serves as a liaison between hospital, com munity and other institutions Utilizes therapeutic co mmunication in daily practice Responsible for employee health and safety, practice issues and troubleshooting after hours Responsible for calling Administrator On Call (AOC) for appropriate issues Responsible for obtaining supplies from Materials Management, Physical Therapy and Pharmacy after hours Know ledge able of process for adding surgeries and other proced ures to schedules after hours Knowledgeable of resources to use to maintain hospital working environment Work s with agencies to obtain per diem staff and to maintain files on agency and PRN staff Serves as clinical expert for troubleshooting patient complaints, care issues, and physician issues Facilitates all patient transfe rs to other facilities Performs frequent and daily rounds at main campus ensuring patient and staff needs a re met What qualifications you will need: Advanced Cardiac Life Spt must be obtained within 30 days of employment start date Basic Cardiac Life Support must be obtained within 30 days of employment start date (RN) Registered Nurse Associate Degree; BSN preferred No
