Qualifications
Provide individual, family, and group interventions to support treatment goals and patient progress. preferred
Develop, implement, and update individualized treatment plans in collaboration with patients, families, and the interdisciplinary treatment team. preferred
Facilitate treatment planning meetings and coordinate care with physicians, nursing staff, and other clinical disciplines. preferred
Assist patients and families with identifying resources and addressing psychosocial, educational, financial, and discharge planning needs. preferred
Monitor patient progress, evaluate treatment effectiveness, and adjust interventions as appropriate. preferred
Facilitate family meetings and support ongoing communication between patients, families, and the treatment team. preferred
Maintain timely, accurate, and compliant clinical documentation, including assessments, treatment plans, progress notes, and discharge planning activities. preferred
Ensure confidentiality and compliance with all applicable regulatory, accreditation, and organizational standards. preferred
