Sentara Norfolk General Hospital is hiring an Inpatient Case Manager/ Social Worker - MSW! This position is eligible for up to a $15,000 sign-on bonus for qualified candidates. Status: Full-Time, permanent position (36 hours OR 40 hours) Standard working hours: 3x12 weekend schedule/ 36 hours: 8:00 AM to 8:30 PM EST, Friday, Saturday, and Sunday OR Saturday, Sunday, and Monday, including a 30-minute lunch break 4x10 schedule/ 40 hours: 8:00 AM to 6:30 PM EST, Friday through Monday, including a 30-minute lunch break Eligible for weekend shift differential The position requires training Monday through Friday from 8:00 AM – 4:30 PM for the first several weeks before transitioning to the full-time weekend day shift.
Responsible for the provision of medical social work services to patients receiving care in the hospital setting. Assesses the social, cultural, environmental, and financial situations, as well as the disposition needs of each referred patient. Collaborates with healthcare team members for the patient and community organizations to identify needs and resources in the hospital and community.
In consultation with the healthcare team develops effective transition plans. Collaborates with the RN Case Manager to ensure a smooth flow of patients through their hospital stay. Functions in one of the following practice settings: Hospitals Only
MLD - Master's Level Degree in Social Work / MSW REQUIRED Certification/Licensure Basic Life Support (BLS) - Certification within 90 days of hire. Case Management (ACM, CCM, CCCTM, CMAC, or CMGT-BC) certification is required within one year of eligibility.

The base pay range for full-time employment is $72,737.60 - $110,323.20 annually. Additional compensation may be available for this role, such as shift differentials, on-call pay, shift premiums, extra-shift incentives, or bonus opportunities. Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs.
Note: If an annual salary is posted, it is based on a full-time colleague working 2,080 hours annually.
