Intake Coordinator, Home Health Services, Full Time First Shift

Begins the pre-admission process for all potential Home Health (HH) admissions received through the acute care Discharge Planner referrals or referrals from physicians or outside providers. Gathers all clinical, social, & insurance related information leading to a comprehensive referral. Takes physicians orders and follows agency policy and protocols.

Assists field staff in managing non direct care activities. Assists agency leadership in performance improvement activities. Participates in Unit Interdisciplinary Team Conferences, performance improvement, and customer satisfaction initiatives.

Performs any other job related duties as requested by the Directors.
