Intake Referral Coodinator - LVN

Plans and coordinates intake and referrals for Hendrick Home Health; ensuring the timely receipt and review of accurate patient information; physician orders; and supporting documentation to facilitate appropriate admissions and quality patient care. Job Requirements M inimum E ducation LVN M inimum W ork E xperience 1 to 3 years of experience as an LVN; preferably with experience in home health. R equired L icenses/ C ertifications CPR (BLS) R equired S kills ; K nowledge ; and A bilities Strong communication skills; verbal and written alike.

Understanding of physician orders and clinical definitions. Knowledge of patient retention; ability to differentiate levels of urgency; and organizational skills. Knowledge of regulatory standards regarding physician offices and home health; specifically in compliance with Medicare.
