Cedar Creek provides evidence-based treatment to adolescents and adults struggling with mental health issues.

As part of our success and innovation in the field we are seeking qualified applicants for an Intake Nurse/Social Worker position. Information is normally obtained through initial clinical phone information, followed by clinical evaluation if appropriate for the client and coordinates with the physician regarding patient care needs. The Intake staff will facilitate this process by assisting the client, family and referral sources. They also provide information to clients, families and referral sources regarding treatment programs, insurance and admission procedures.
