in this role: Coordinating the provision of patient care services related to such aspects as psychosocial and needs assessment, crisis intervention, and emotional support Research and advocate for appropriate public assistance resources for clients Referral to community resources, assistance with socio-economic problems Communicate with client’s care team Provide crisis intervention (referral to DCF, provider, etc as needed) Coordinate treatment planning with specialists and outpatient providers as needed Evaluate current patients for appropriate social work services Judge relative costs/benefits of potential social services and health care actions and choose the most appropriate one.

Master of Social Work from CSWE Accredited Institution (Required)
