Promotes knowledge of oncology and its effects for patients, support partners, and care providers. Meets with patients and support partners after diagnosis to provide counseling and referral services specific to their needs. Collaborates with the multidisciplinary team to provide ongoing support for patients and oversee the plan of care.
Educates and collaborates with the multidisciplinary team to plan and participate in quality initiatives and research projects. Reviews patient information and assess patients for emotional needs and risk factors. Serves as a customer service advocate for patients and support partners.
Serves as the primary point of contact for providers and patients requiring the Oncology Licensed Social Worker (OLSW). Works with Navigators to provide Survivorship Care Plans to patients. Performs patient and family evaluations and histories, providing psychosocial support through individual or family counseling as needed.
Acts as a resource to Infusion/Oncology Practices for patient needs and collaborates with patients to identify potential barriers to achieving oncology treatments and set health goals for quality of life.
