Citizens Memorial Hospital (CMH) is hiring a License Practical Nurse (LPN) to join our Care Transitions Department in Bolivar, Missouri — someone who leads with skill, compassion, and a drive to positively impact every patient and family we serve. Position Details Status: Full Time Work
Days Hours: 8:00a.m-4:30p.m What Your Day Looks Like: Care Transitions answers and returns patient phone calls for CMH primary care clinics. The department is made up of two teams, schedulers and triage nurses, who work closely together. As a Licensed Practical Nurse or Registered Nurse, you will be: Answering and returning patient calls for triage and nursing advice.
Relaying information and results between patients and their primary care provider. Assisting with facilitating medication refills, authorizations and questions. Identifying and connecting patients to organizational resources.
Completion of an accredited program (Licensed Practical Nurse or Registered Nurse)
Patient Care
Organization, able to work in a fast-paced environment, multitasking, patient care, strong written and verbal communication. Licensure/Certification: Active Missouri License as a Licensed Practical Nurse or Registered Nurse. Physical
