OverviewUnder supervision of a professional nurse, renders nursing care to patients and performs related work as required and within he limits of training.

ResponsibilitiesCollects data from patient, family or significant others and records on Initial Data Base formContributes to the formulation and updating of plan of care and implements plan of care under the direction of the Advanced Practice ProviderGives direct nursing care to assigned patients (e.g., medications, treatments, dressing changes, TPR) according to established standardsRecognizes changes in patient condition and responds appropriately, notifying charge nurse as neededDocuments pertinent information regarding patient's responses and nursing interventions in patient's medical recordAdministers medication according to hospital policiesApplies principles of infection control and safety in the performance of patient careCommunicates effectively with charge nurse and other member of the Health Care Team regarding patient's needs and/or changes in conditionResponds appropriately in crisis situationReports significant unit problems to the Advanced Practice ProviderPatient care responsibilities are delivered with a knowledge of patient growth and development and are appropriate to the ages of the patients servedMaintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental, and infection control standards.

Promotes adherence to the Patient's Bill of RightsSchedules and coordinates patient appointmentsObtains pre-authorizations for diagnostic exams and procedures QualificationsGraduation from an approved Practical Nursing program. Current NYS licensure as an LPN required. BCLS certification; venipuncture; EKG certification. 1 year of LPN experience in an ambulatory health facility preferred.

Good computer skills required. Good oral communication and interpersonal skills required. Speaks, reads, writes English required to the extent of the position.
