We focus on the patient returning home at their maximum level of abilities. As a nurse you have frequent interactions with the patient and family providing immediate care and education for a successful transition home. The nurse must be able to assess patient condition, administer medications, change dressings, monitor vital signs, keep records and provide patients and families with support and education. The Medical-Surgical nurse maintains a wide array of medical care knowledge in order to care for a diverse group of patients. Job Requirements Education

• BLS must be obtained within 90 days Licensed Practical Nurse Job Summary The Licensed Practical Nurse working in the Transitional Care Unit is responsible to provide direct care of the adult/geriatric patient experiencing prolonged general medical or post-operative recovery. The LPN provides care based on the patients’ needs, which includes medication administration, dressing changes, monitoring of vital signs and skin integrity, documentation in the medical record, and providing patients and families with support and education.
