Licensed Practical Nurse - Home Health Care/Hospice

To provide adult and geriatric home health care with the supervision of the primary nurse that is collaborative with all members of the interdisciplinary team. Qualifications JOB

Under the supervision of a RN or provider, the LPN uses sound nursing judgment in the provision of nursing care. Performs a variety of both technical and routine nursing services. Collaborates with health care teams to implement the nursing care plan.

Demonstrates clinical competence. Qualifications - External EDUCATION/EXPERIENCE REQUIRED: Currently in last semester of or graduated from an accredited LPN program. Effective written, verbal and interpersonal communication skills.

Ability to problem-solve. Critical thinking skills and judgment necessary in the delivery of patient care. Knowledge about the care of neonatal, pediatric, adolescent, adult, and geriatric patients Exceptional customer service skills.

Basic computer knowledge.

LPN experience; Women and Children Services preferred.
