Typically, travel is limited to one to two locations per day, with up to four sites in Rochester, NY and one in Newark, NY. Mileage reimbursement is provided.
The Licensed Practical Nurse is responsible for providing direct patient care, usually under the direction of a Registered Nurse, in accordance with established plans.
- Conducts and documents a thorough observation of each patient’s medical status upon admission and throughout the patient’s course of treatment.
- Assists in the implementation of the patient’s treatment plan under the direction of the RN.
- Provides routine nursing services for patients as directed by the RN.
- Complies with the evaluation, treatment, and documentation guidelines of the department.
- Completes required documentation in an accurate and timely manner.
- Provides regular patient status updates to the appropriate personnel.
- Attends and participates in department/facility meetings as required.
- Acts in compliance with hospital, regulatory, and professional standards and policies.
- Maintains clinical competency by pursuing continuing education opportunities.
- Performs other duties as assigned. REQUIRED LICENSURE / CERTIFICATION SKILLS Graduate from an accredited School of Practical Nursing. Current LPN licensure in State of New York. Basic Life Support (BLS) Certification from the American Heart Association (AHA).
1 year experience 1 year of experience working with a frail or elderly population preferred.

Moderate - Clinical - Moderate clinical roles are patient facing roles that require continual movement, including standing, walking, and lifting 20–50 lbs., along with pushing and pulling medical equipment and assisting with patient positioning. Employees perform frequent bending, twisting, and reaching while completing fine motor tasks in busy or confined clinical environments. High sensory acuity is essential for monitoring patient status and responding to alarms or safety concerns.
Emotional and cognitive demands are substantial, involving decision-making under pressure, responding to emergencies, and maintaining sustained focus. Environmental exposures include infectious agents, bodily fluids, chemicals, and radiation. TB testing, mask-fit testing, and physical exams are common requirements.

$28.51 - $34.68 CITY: Rochester POSTAL CODE: 14609 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
