This is performed in accordance with physician orders and plan of care and in accordance with agency policies and procedures.

-Have a current Pennsylvania Practical Nurse License -Have previous Home Health experience (preferred) -Have a valid PA driver's license and reliable transportation -Have 2 years of LPN acute care or long-term care experience -Have excellent time management skills -Have strong verbal and written communication skills -Have ability to exercise initiative and independent judgement -Have ability to work independently and within a multidisciplinary team -Have ability to pass PA Child Abuse History Clearance -Have ability to pass Criminal Background Check -Have ability to meet physical requirements of position WHAT WE OFFER: Professional Development Supportive and Experienced Peers
