Licensed Practical Nurse (LPN) Home Health

The Licensed Practical Nurse provides direct patient care through personal accountability for care needs of patients and families as demonstrated through treatments, administration of medications and other specified assignment under the direction of a Clinical Nurse. The LPN understands Home Health and works with the interdisciplinary treatment team to provide the plan of care. The LPN understands the needs of the organization and supports the mission, values, and management of patient care services.
