What You’ll Do: We’re seeking a dedicated and compassionate Licensed Social Worker to join our team and play a critical role in supporting patients and families throughout their healthcare journey. This is more than just a job—it’s an opportunity to impact lives by providing emotional support, coordinating resources, and advocating for those in need. Provide direct care, counseling, and crisis intervention to patients and their families across all age groups.
Support patients during discharge planning, ensuring a smooth transition of care and access to community resources. Serve as a key member of a multidisciplinary care team, contributing to treatment planning and decision-making. Coordinate referrals to appropriate internal and external support services.
Promote and participate in patient and staff educational programs. What We’re Looking For:
Bachelors or Master’s degree in Social Work from an accredited program.
Current Social Work license through the Ohio Counselor, Social Worker, and Marriage & Family Therapist Board.
CPR certification (BLS - Basic Life Support).
At least 1 year of professional social work experience in a healthcare or related setting. Ability to handle confidential information and make thoughtful, independent decisions in a fast-paced environment. Why Caregivers Stay?
