✓Requirements
Licensure
✓Current, unrestricted Social Worker license or LMHC in the state of Hawaii
Certifications
CCM (Certified Case Manager) preferred
Qualifications
✓2+ years of social work or similar experience
✓Experience working directly or collaborating services for long-term care, home health, hospice, public health or assisted living
✓Intermediate experience working with MS Word, Excel and Outlook
✓Ability to travel in assigned region to visit Medicaid members in their homes and/or other settings, including community centers, hospitals or providers' offices
✓Current access or ability to obtain internet access via a landline
✓Driver's license and access to reliable transportation
✓All employees working remotely will be required to adhere to UnitedHealth Group's Telecommuter Policy.
Case Management experience preferred
Experience with utilization review, concurrent review and/or risk management preferred
Experience with electronic charting preferred
Experience with arranging community resources preferred
Field-based work experience preferred
Background in managing populations with complex medical or behavioral needs preferred
Background in mental health or experience working with serious mental illness preferred
Bilingual in Cantonese or Mandarin preferred
