Make a Difference The Life Skills Clinician (LSC) will engage with individuals and families to improve their health and wellness, live self-directive lives, and strive to reach their full potential. The LSC monitors and provides care coordination and life skills training and development, in collaboration with the care team, to youth and/or adults with co-occurring mental health and/or substance use disorders- including those with serious and persistent mental illness.
Bachelor's Degree required Exceptional Skills and Qualifications P atients First Collaborate as part of a multidisciplinary care team by supporting the comprehensive psycho-social assessments. Ability to help clients regulate emotions. Document services provided in Electronic Medical Record (EMR) based on policies and procedures.
When necessary, will transport clients in personal vehicles as well as, when necessary, transport client medications to clients.
