The Clinical Care Coordinator II demonstrate the competency to provide patient care according to the age-specific population needs and assist in the management of patient care in the physician office setting under the direct supervision of a provider. This position is with FPG Nurse Triage Team - will work at FPG's Centralized Access Center on University Parkway in Sarasota. *Must be comfortable working in a call-center environment and doing phone calls for entire shift.

This position does not provide direct patient care. The LPN will take calls from established FPG patients and follow protocols for patient symptoms and offer guidance based upon strict adherence to established protocols. The goal is to provide excellent patient care and provide real time clinical guidance.

This team will work closely with the Clinical Excellence and Operations team to ensure seamless patient care experiences in collaboration with providers and develop effective communication with the provider office care teams. Required Qualifications - Require Basic Life Support/CPR certification; or must be obtained within thirty (30) days of hire/transfer date. - Require graduate of an accredited school of nursing or a diploma program.
