An LPN shall communicate if the patient’s condition deteriorates or if there is a significant change in condition, the patient is not responding to therapy, the patient becomes unstable or the patient needs immediate assistance. with a registered nurse, physician, and the patient’s health care team members to seek guidance with patient care.
An LPN evaluates and collects data as allowed by the licensed practical nurse scope of practice as defined by the Pennsylvania/New Jersey Nurse Practice Act. Work closely with the registered nurse to assure the implementation of safe, quality care.

Medication Administration and Prescription Knowledge of various medications used to treat patients, ability to apply this knowledge to the administration of medications as allowed by the licensed practical nurse scope of practice defined by the state of practice Nurse Practice Acts Discharge Planning Knowledge of the patient's post-discharge needs and the ability to partner with the patient and family/designated care partner(s) in reinforcing a discharge plan and arranging for follow-up care. Evaluation and Diagnosis Knowledge of various tools and techniques for patient observation, evaluation and nursing diagnosis or clinical judgement, ability to explore and analyze reasons behind symptoms and generate resolutions according to available drugs, facilities and techniques as allowed by the licensed practical nurse scope of practice defined by the state of practice Nurse Practice Acts Health Information Documentation Knowledge of the relevance of health information and appropriate documentation methods, ability to record medical histories, symptoms, vital signs, medications, diagnoses, treatment plans, procedures, tests, and medical results in written or electronic form.
