You’ll need
Licensure
✓Current LPN license in the state of practice.
✓Must be a licensed driver.
Certifications
✓Possesses and maintains current CPR certification.
✓must be able to perform CPR alone.
Education
✓Graduate of an accredited practical nursing program; Consideration will be given for comparable education experience and successful licensure.
Qualifications

✓Understands Hospice philosophy
✓issues related to death/dying and grief.
✓Complies with Unity Health Hospice Health Status requirements.
✓Has ability to provide good written documentation in a timely manner.
✓Is flexible and cooperative in fulfilling role obligation and has ability to work within an interdisciplinary team.
✓Employee has significant impact on patient welfare. Effects or errors in determining patient condition or administering medication could be critical.
✓Employee is frequently to continuously alone with patient and must be able to handle emergencies without assistance.
✓Employee must be able to assist falling or fallen patient
✓Employee may be required to drive at night and/or in hazardous conditions; must be able to follow written/verbal instructions and read a map. Employee must independently schedule patient visits in timely and efficient manner; checking in with office daily to update schedule.
✓Employee has regular contact with agency staff members, physicians, hospital or medical center staff, general public.
✓Employee regularly handles confidential information.
Prefer one year experience in a related field. preferred
