The Behavioral Health LVN Intake Coordinator is responsible for providing high-quality patient care and displaying excellent assessment skills in the areas of mental health and chemical dependency. The intake coordinator will have experience in Crisis Intervention, Triage of patients in crisis, acting as liaison to physicians requiring information to make clinical decisions. The LVN Intake Coordinator will obtain admission and medical orders from physicians for patients who come into DHR-BH.
LVN Intake Coordinator must be knowledgeable in reviewing labs to assure clients are medically stable. LVN Intake Coordinator will be knowledgeable on normal parameters of vitals, lab values, and how to escalate if any abnormal values and medical emergencies and receiving/giving report for incoming/outgoing medical transfers. LVN Intake Coordinator will be responsible for skin assessments, escorting patients to units and assisting in medical concerns.
The intake coordinator will demonstrate skill in determining and documenting medical necessity for admission; have an understanding of TJC and CMS guidelines as they relate to access to care in a mental health facility. The Intake Coordinator is expected to function well as a member of the interdisciplinary team and participate in community relation activities, performance improvement activities and hospital sponsored events.
POSITION EDUCATION/
Licensed LVN required Related experience can be substituted for degree for a candidate currently enrolled in an acceptable Bachelor’s Degree program. Current BLS Certification required Abuse, Neglect, and Exploitation (ANE) completed prior to orientation completion and annually thereafter. CPI required within 30 days of hire/transfer One year clinical experience in a position primarily responsible for assessment, crisis intervention and disposition of mental health patients in crisis.
Experience working in a hospital setting or emergency room and acting as a liaison between physicians a plus. Working Conditions: The individual spends over 95% of his/her time in an air-conditioned environment with varying exposures to noise. There is protection from weather conditions but not necessarily from temperature changes. The position does have low exposure to malodorous, infectious body fluids from patients and some minimal exposure to noxious smells from cleansing agents.

Occupational Exposure: This position has minimal to no exposure to blood, body fluids, or tissues and is an OSHA Category III (although situations can be imagined or hypothesized under which anyone, anywhere, might encounter potential exposure to body fluids).
