- 2400. Responsibilities Assess, plan, implement, and evaluate patient care using the nursing process. Provide skilled nursing care to patients with tracheostomies, ventilators, feeding tubes, and other specialized medical needs. Monitor and document patient status, identifying changes in physical or psychosocial condition. Administer medications and treatments as ordered by the physician. Collaborate with family members, caregivers, and the healthcare team to ensure optimal patient outcomes. Maintain accurate and timely documentation.
