• JOB SUMMARY Under the supervision of the Administrative Nurse Manager, the Sr. Licensed Vocational Nurse utilizes the nursing process to assess and identify the individual needs of patients through observation of disease symptoms, emotional state, laboratory reports, cultural and socio economic background and response to medical regimen and treatment. Additionally, they are required to learn and apply the Patient
• Centered Medical Home Standards and Ten Building Blocks of Primary Care. They are responsible for providing nursing care to patients in the ambulatory setting. The Sr. LVN will work as an important member of the care team, working closely with the RN and the physicians to deliver all components of nursing care, individualized to meet the patient’s physiological and psychosocial developmental level and needs. The Sr. LVN will be responsible for chart abstraction. The Sr. LVN consistently projects a professional and courteous demeanor, and shall comply with the Medical Center House and Telephone Standards. In addition, they will have clinical responsibility for administration of medications/vaccines, lab result review and monitoring, treatments and procedures pertinent to pediatric & adult primary care, such as ear irrigations, wound care and urethral catheterization, as well as patient education. The Sr. LVN must have flexibility to cover multiple UCSF medical center clinic sites. This individual will utilize their good customer service and communication skills on a daily basis via phone/electronic contact, mail correspondence, and personal contact with both internal and external customers. They will utilize computer skills in ApeX and other UCSF computerized programs. This individual supports the standards of the policies and procedures of the practice, and is responsible for maintaining current knowledge of pharmacology and competency in modes of administration, and complies with all Ambulatory Services and Medical Center policies and procedures for infection control, safety, and administrative and clinical practice. They are required to adhere to the Telephone and House Standards as adopted by the Medical Center, as well as demonstrating UCSF’s PRIDE values and the AIDET fundamentals of communication in all patient care and stakeholder interactions. Recognizes and reports specific physical and emotional conditions/needs/concerns of the patient to appropriate members of the health careteam. Alerts a provider of unusual observations of patients to facilitate immediate attention, e.g. chest pain, shortness of breath. Learns how to respond in a code blue scenario and understands use of emergency equipment. Reports emergency or unsafe patient care conditions on the Incident Reporting System and to the management team. 5% Planning Works cooperatively and participates withthe health careteamlet to discuss patient’s alternatives to health care services and to develop short and long range goals to meet the patient’s needs. Plans as appropriate for the needsof patient usingknowledge of physical and emotional processes as well as cultural and socioeconomic backgrounds. Identifies appropriate resources (e.g., family, significant others, monetary, community organizations, online or written teaching tools) in planning for patient care. 40% Implementation/Evaluation Provides individualized nursingcare which responds to the growth and developmental needs of patients and demonstrates an understanding of the range of treatment required by the patient. Provides forpatient’s physical and emotional privacy. Prepares for and instructs patients about specific diagnostic and therapeutic procedures and treatments. Assists as needed with diagnostic and therapeutic procedures and treatments. Thisincludes vaccines, injections, wound care and phlebotomy. Obtains specimens and performs Point of Care Testing as directed. Performs QA/QCon CLIA waivedtests and reports any tests out of QA range to supervisor. Uses appliances and equipment correctly and maintains safe practice. Evaluates and documents patients’ responses to nursing care in the electronic medical record, including pre/post procedures and treatments. Uses knowledge of cognitive, physical, emotional and chronological maturation processin evaluating care for all ages of patients. Administers and documents the administration of medication while maintaining current knowledge of pharmacology and competency in modes of administration. Documents telephone messages and chart notesin a thorough and grammatically correct manner. Responds appropriately in emergencies. Is knowledgeable about the contents of the code blue cart and/or the emergency kits,and can initiate and assist in CPR.Understands the procedures for medical emergencies, code blue and other emergency/disaster situations. Participates in safety drills. Complies with all Ambulatory Services and Medical Center policies and procedures for infection control, safety, administrative and clinical practice. Complies with activities mandated by TJC/Title 22/NCQA. Completes annual competency testing as specified by the individual practice. 20% Coordination/Continuity/Communication Organizes activities based on the needs of patients while maintaining the effective flow of patients through the practice. Adjusts clinical support activities to reflectchanges in priorities during practice sessions. Maintains open communication and facilitates continuity of care for patients/and or family withmembers of the health care team through phone contact, verbal interaction. Maintains accurate, records and documentation which include patient complaints, nursing observations, interventions and care plan, nursing actions/teaching performed and patient response to the action. Obtains, records, and conveys information, clearly and concisely. Demonstrates courtesy, commonsense, and follow-through in all telephone/electronic communications, maintaining the Telephone Standards as established by the Medical Center. Completes all taskswithin assigned InBasket to include cross-coverage of InBaskets of other teamlets and other practices across primary care. Fosters/maintains positive public relations (patients, family, visitors and staff). Uses effective communication skills with patientsand staff; demonstrates proper telephone techniques and etiquette; shows sensitivity to differences of culture; demonstrates a positive and supportive manner in which patients perceive interactions as positive and supportive. Minimum 3 years of experience as a Licensed Vocational Nurse in an ambulatory setting Knowledge and abilities essential to the successful performance of the duties assigned to the position, including demonstrated excellent communication skills, problem-solving ability, and high comfort level with learning new technology/systems. Demonstrated ability to provide highly personalized patient care Demonstrated ability to work as an integral part of a patient care team.

Demonstrated communication skills, both verbal and written. Strong customer service orientation. Ability to administer immunizations to adult patients Basic computer/keyboarding skills.

Prior experience with electronic systems/records. Demonstrated ability to work in a fast-paced environment. Trained and licensed to perform phlebotomy

Spanish, Chinese, Russian language skills a plus Familiarity with the principles of a Patient-Centered Medical Home environment Experience working in a primary care internal medicine office . Licenses/Certifications: Current LVN License in the State of California Active American Heart Association BLS Certificate
