About the role

The medical social worker provides direct social work intervention, transition planning and counseling to patients and their families. The medical social worker assesses the patient’s social, emotional, financial, cultural and family situation and interprets this information to the physician, nursing staff and other care team members to facilitate appropriate transition planning.
